Event Description: 8:30am – 9:00am: Breakfast & Conversation 9:00am – 11:00am: Program Since 2012, Connecticut has been building a system that enables us to identify, assess, prioritize and match the most vulnerable population experiencing chronic homelessness to housing resources within a short period of time. While we have been able to greatly reduce the number of those experiencing chronic homelessness, we understand that this is an ongoing and complex problem that needs continued attention. This IForum will explore how we integrate mainstream systems such as behavioral health, criminal justice, and health care systems, and assure that there are pathways to address issues related to affordable and supportive housing. Adequate case management and health services to promote housing stability for those experiencing chronic homelessness will be discussed. Introductions by: - Commissioner Evonne Klein, CT Department of Housing - Commissioner Miriam Delphin-Rittmon, CT Department of Mental Health and Addiction Services Keynote presentation by: - Terri Power, Senior Program Manager---Returning Home Ohio, CSH Returning Home Ohio is an initiative aimed at preventing homelessness and reducing recidivism among individuals returning to Ohio’s communities from state prisons. Panel discussion exploring the intersection of feeder systems and innovative approaches to ending chronic homelessness, including: - Moderator – David Gonzalez Rice, Connecticut Coalition to End Homelessness - Kevin Kane, Chief State’s Attorney - Dr. Kathleen Maurer, Medical Director, CT Department of Corrections - Fred Morton, CT Department of Mental Health and Addiction Services - Staci Peete, Norwalk Hospital - Kathy Flaherty, CT Legal Rights Project Presentation exploring sustainability of resources to prevent future chronic homelessness by: - Steve DiLella, CT Department of Housing


Directions:
From Boston and the East: I-84 West to exit 48 (Asylum Avenue). When you exit, get into the center lane. At the end of the exit, take a wide right turn from the center lane onto Asylum Ave. Immediately bear left at the fork onto Farmington Avenue. Follow Farmington Avenue to the first light, which is Broad Street. Turn left onto Broad Street and continue to Capitol Avenue (gas station will be on the corner). Turn right onto Capitol Avenue. At the first light, turn left onto Lawrence Street. (Red Rock Café will be on the corner) The Lyceum is about half way down the first block on the right side. It is a large, brick three-story building. Parking is available in the lot just before The Lyceum From New Haven and the South: Take I-91 North to I-84 West. Stay in the right lanes and follow directions from Boston above. From Western Massachusetts and the North: Take I-91 South to I-84 West. Stay in the right lanes and follow directions from Boston above. From Danbury and the West: Take I-84 East to Exit 48B, Capitol Avenue. Turn right off the exit to Capitol Avenue. Turn left at the third light (you’ll see the Red Rock Tavern on the corner) onto Lawrence Street. The Lyceum is the distinctive brick building, the sixth building on the right. Park in the lot just before the building or turn right around the corner for our additional lot.
